WHAT DO I NEED FOR MY APPOINTMENT?
Please bring your photo ID, health insurance and payment for services. If this is your first time visiting us, there will be forms that need to be completed, so please arrive 15 minutes prior to you appointment time. You can also complete the forms prior to your appointment by printing out and completing the forms below and bring them to your appointment.
HOW DO I GET MY MEDICAL RECORDS?
Our office only handles paper medical files and therefore can either fax or copy your medical records for a fee. We require a signed authorization and payment before we can release of those records to you or any other authorized third party.
